PAYROLL TITLE

Pharmacist – Specialty Care

EDUCATIONAL/BILINGUAL INCENTIVE:

Incumbents may be eligible for an educational incentive of 2.5%, 3.5%, or 5% based on completion of an Associate’s, Bachelor’s, or Master’s degree that is not required for the classification. Incumbents may also be eligible for bilingual incentive depending upon operational need and certification of skill.

DUTIES MAY INCLUDE, BUT ARE NOT LIMITED TO THE FOLLOWING:
  • Lead assigned patient service lines, clinical area, and therapeutic programs as an active member of a multidisciplinary team and collaborate with healthcare providers.
  • Perform patient assessments and monitor the effects of drug therapy using clinical symptoms, evaluating laboratory data, and other relevant clinical diagnostic techniques.
  • Critically analyze, evaluate, and prepare recommendations of medical literature in relation to medication therapy.
  • Design and implement stewardship activities and restriction/surveillance programs.
  • Track and evaluate assigned pharmacy programs for operational, quality, and financial efficiency and routinely benchmark against local and national best practices.
  • Actively identify practice-related issues which require evaluation, facilitate clinical research projects, quality improvement initiatives, or develop and administer healthcare provider education; summarize results of evaluation/projects and submit recommendations to the Pharmacy Director and/or Pharmacy and Therapeutics Committee.
  • Assist in establishing drug monographs for formulary assessment.
  • Develop and oversee policies and procedures for drug purchasing, usage, distribution, and control including specialty medication.
  • Assure pharmacy is an integral part of the health‑care delivery system and facilitate enhancement and expansion of pharmacy services/programs.
  • Participate in performance improvement activities including drug utilization evaluation.
  • Facilitate experiential education and practice advancement by maintaining proficiency in receptor roles and emulating education/mentorship skills.
  • May dispense medication and perform duties as a Pharmacist as necessary.
  • Perform other related duties as required.

These are entrance requirements to the exam process and ensure neither continuance in the process nor placement on an eligible list.

EDUCATION / EXPERIENCE
  • Pharmaceutical degree (Bachelor of Science in Pharmacy) from an accredited school of pharmacy or its equivalent.
AND
  • A combination of three (3) years of experience as an inpatient staff pharmacist with exposure to chemotherapy,
    OR
  • one (1) year of specialized clinical pharmacy experience in inpatient oncology or outpatient infusion center OR
  • Successful completion of a pharmacy residency program in clinical pharmacy.
NECESSARY SPECIAL REQUIREMENTS
  • Must possess and maintain a current valid license as a Pharmacist in the State of California.
  • Physical ability requirement: must be able to lift 25 pounds.
DESIRED
  • Possession of an Oncology Pharmacy Specialty Certification (BCOP).
KNOWLEDGE, SKILLS, AND ABILITIES
  • Thorough knowledge of prescription filling activities.
  • Sterile and non‑sterile compounding.
  • Sterile compounding of non‑hazardous and hazardous medication.
  • Pharmacology and pathophysiology related to management of oncology/hematology patients.
  • Professional standards and practices.
  • Federal, State Board of Pharmacy, The Joint Commission, and Department of Public Health laws, regulations, and guidelines pertaining to patient care.
WORKING ABILITY TO
  • Operate a computer.
  • Perform pharmaceutical mathematical calculations.
  • Research, read, interpret medical and pharmacy literature.
  • Communicate clearly and effectively.
  • Review and verify medication orders accurately and timely through electronic health record.
