Responsibilities

• Provide care and services for hospice patients in homes, assisted living facilities, and hospitals.
• Complete admissions and conduct patient visits as needed.
• Answer calls from patients, families, and staff.
• Conduct symptom management, visit for deaths, and admissions during on‑call shifts.
• Serve as an advocate for hospice services and Suncrest Hospice.
• Work after‑hours on‑call for seven nights on, seven nights off (5 PM‑8:30 AM).

Qualifications

• Current RN licensure (in good standing) in state(s) of practice with at least one year of experience.
• Experience with admissions and on‑call duties preferred.
• Strong teamwork skills – collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work independently with phone, email, and documentation tasks.
• Excellent time‑management skills for scheduling visits, meetings, and triage.
• Familiarity with electronic medical record systems.
• Valid driver’s license, reliable transportation, and auto insurance for travel across the service area.
