Pediatric Nurse Practitioner at Santa Rosa Community Health
Job SummaryAs a member of the Patient Centered Medical Home Team, Pediatric Nurse Practitioners work under the clinical supervision of the Medical Director to provide comprehensive primary health care, prevention, and health education to patients seeking care at Santa Rosa Community Health.
Responsibilities- Take complete medical histories, perform physical exams, and make assessments and diagnoses for teen and pediatric patients.
- Initiate, review, and revise treatment and therapy plans, including developmental assessments, well‑visit care, family planning, and childhood vaccines.
- Maintain adequate documentation of all patients seen, including follow‑up and referrals, in the electronic medical record.
- Manage stable phases of chronic illnesses in consultation with the attending physician or Medical Director as needed.
- Order and interpret laboratory reports and diagnostic imaging.
- Assess mental health and provide treatment as needed in individuals and families.
- Coordinate health management with physicians and other community agencies.
- Assure quality and safety through a care planning process, evidence‑based medicine, and active patient participation in decision‑making.
- Recognize emerging community health problems and help initiate interventions through community action.
- Meet with the Medical Director or Attending Physician regularly to review charting and medical care provided.
- Seek consultation on difficult patients as described in mid‑level protocols in the Medical Administrative Protocols Manual of SRCH.
- Participate in ongoing training and quality improvement measures.
- Current unrestricted California license as a Pediatric Nurse Practitioner.
- At least two years’ experience in a community health center, inpatient, emergency room, or urgent care setting (preferred).
- Ability to work independently with physician telephone backup.
- Current CPR certification.
- Fluency in Spanish and English (speak, read, and write) preferred.
- Knowledge of compliance issues within the community clinic environment.
- Willingness to report any incident that is unusual or incompatible with accepted clinic procedures.
- Compliance with HIPAA policies.
- Strong interpersonal skills and sensitivity with persons of various social, cultural, economic, and educational backgrounds.
- Proficiency with Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
- Strong organizational skills with ability to prioritize projects, manage multiple tasks, and meet deadlines.
- Strong written and verbal communication skills.
- Ability to work independently and as part of a team.
- Good judgment, problem‑solving, and decision‑making skills.
