Fox Home Health is seeking a Licensed Vocational Nurse (LVN) for a full‑time, on‑site position located in Santa Rosa, CA. The LVN will provide high‑quality nursing care to patients in their homes, performing duties such as administering medications, monitoring vital signs, assisting with daily living activities, and educating patients and their families on care plans. The LVN will follow individualized care plans, document patient progress, and collaborate closely with the healthcare team to deliver patient‑centric care.

• Current and valid Licensed Vocational Nurse (LVN) license in the state of California
• Strong clinical skills in administering medications, wound care, and monitoring patient conditions
• Knowledge of care planning, healthcare documentation, and compliance with health regulations
• Exceptional communication, patient education, and interpersonal skills
• Ability to work empathetically with diverse patient populations and prioritize patient needs
• Reliable transportation and a valid driver's license for commuting to home visits
• Previous experience in home healthcare is a plus
• CPR certification required
• Organizational skills and ability to maintain accurate patient records
