About The Position

The Department of Health Services (DHS) Public Health Division is recruiting a bilingual (English/Spanish) Public Health Nurse II. Nurses partner with clients during pregnancy and conduct home visits until the child's second birthday, providing education on pregnancy, childbirth, breastfeeding, child development, and nutrition. The nurse manages a caseload of approximately 25 primary clients and their children, ensuring they receive individualized assessments, ongoing follow‑ups, and health education.

Responsibilities
  • Conduct visits in clients’ homes or at other agreed‑upon locations, typically checking in with multiple families per day and collaborating with families to develop and follow individualized action plans.
  • Conduct routine comprehensive assessments.
  • Actively coordinate and follow up on referrals to other health and social service providers and work with local agencies and community organizations to meet clients’ needs.
  • Deliver culturally responsive health and parenting education.
  • Maintain accurate, detailed, and up‑to‑date records of all client interactions, assessments, action plans, and referrals.
  • Attend team meetings and training sessions, and participate in regular outreach activities.
  • Conduct outreach with referral sources to identify new clients (e.g., attending weekly Neonatal Intensive Care Unit (NICU) rounds at area hospitals).
Qualifications
  • Possession of a California State Public Health Nursing Certificate.
  • One year of public health nursing experience obtained after receipt of a California Public Health Nursing Certificate.
  • Valid driver’s license (required for field visits).
Skills
  • Fluent bilingual (English/Spanish) skills.
  • Knowledge of maternal‑child nursing, community, clinic, and public health settings.
  • Experience with trauma‑informed practices and working with individuals impacted by substance use, homelessness, mental health challenges, and intergenerational trauma.
  • Ability to work independently and collaboratively with a multidisciplinary team.
  • Previous experience as a Nurse Home Visitor, Public Health Nurse, or similar.
  • Interest in fieldwork and the ability to work with families of diverse customs and traditions.
  • Respect for clients’ strengths and the ability to build positive relationships.
  • Openness to continual learning.
