Overview
This recruitment is promotional only. You must presently be a full or part-time regular or probationary County of Sonoma, Community Development Commission, Sonoma County Fairgrounds, or Sonoma County Water Agency employee in order to apply for this position.
The Human Services Department (HSD) has requested a department promotional certification to fill an Adult and Aging Social Work Supervisor position. Only qualified permanent and/or probationary Human Services Department employees on the eligible list may be eligible for referral to the department for the advertised position. This employment list may also be used to fill future full-time positions as they occur during the active status of the list.
Responsibilities- Oversee day-to-day operations of the Information and Assistance Program, the core services of the Aging and Disability Resource Hub (ADRH), and other work that supports a “no wrong door” system of care.
- Supervise a team of Adult and Aging Social Workers and a Social Work Assistant who receive a high volume of calls from the community related to older adult needs and resources.
- Collaborate across programs to design and develop systems and practices that strengthen service delivery, improve efficiency, and advance a coordinated, person-centered system of care.
- Plan, organize, and support staff work related to ADRU core functions, including Information and Assistance (I&A), Options Counseling (OC), Short-Term Services Coordination (STSC), Transitions Coordination (TC), and Outreach.
- Provide consultation, coaching, and supervision on complex cases, screening, triage decisions, casework, protective concerns, and clinical judgement.
- Lead unit meetings, individual supervision, case conferences, and cross-program coordination meetings.
- Serve as a liaison with internal programs and external partners to promote coordinated access to services.
- Identify trends, recurring issues, and systemic barriers, and provide recommendations to management.
- Support program implementation, staff training needs, and continuous quality improvement efforts.
- Experience supervising, training, and/or coaching staff.
- The ability to manage difficult or challenging conversations with personnel, clients, or others in the community.
- An understanding of aging and disability service systems, “no wrong door” access models, community-based services and support, and the psychosocial and functional needs of older adults, people with disabilities, and their caregivers.
- A working knowledge of clinical supervision, crisis intervention, de-escalation strategies, and person-centered, trauma-informed, culturally responsive practices.
- Experience working in a call center or high-volume service environment.
- The ability to manage competing priorities in a fast-paced setting.
- Experience collaborating with healthcare systems, community-based organizations, and multidisciplinary partners.
- Knowledge of crisis intervention techniques and risk assessment.
- Experience working with community agencies and advocating for older adults and people with disabilities.
- Excellent interpersonal and communication skills, with the ability to interact with a diverse client population.
- A Master’s degree in Social Work or a closely related behavioral health or human services field is highly desired, but not required.
