Position Information

This recruitment is promotional only. You must presently be a full‑ or part‑time regular or probationary County of Sonoma employee in one of the following agencies:
Community Development Commission, Sonoma County Fairgrounds, or Sonoma County Water Agency. Only qualified permanent or probationary Human Services Department employees on the eligible list may be eligible for referral to this advertised position. The employment list may also be used to fill future full‑time positions as they occur during the active status of the list.

Responsibilities
  • Planning, organizing, and supporting staff work related to ADRU core functions, including Information and Assistance (I&A), Options Counseling (OC), Short‑Term Services Coordination (STSC), Transitions Coordination (TC), and Outreach.
  • Providing consultation, coaching, and supervision on complex cases, screening and triage decisions, casework, protective concerns, and clinical judgment.
  • Leading unit meetings, individual supervision, case conferences, and cross‑program coordination meetings.
  • Serving as a liaison with internal programs and external partners to promote coordinated access to services.
  • Identifying trends, recurring issues, and systemic barriers, and providing recommendations to management.
  • Supporting program implementation, staff training needs, and continuous quality improvement efforts.
Qualifications
  • Experience supervising, training, and/or coaching staff.
  • The ability to manage difficult or challenging conversations with personnel, clients, or others in the community.
  • Understanding of aging and disability service systems, “no wrong door” access models, community‑based services and support, and the psychosocial and functional needs of older adults, people with disabilities, and their caregivers.
  • Working knowledge of clinical supervision, crisis intervention, de‑escalation strategies, and person‑centered, trauma‑informed, culturally responsive practices.
  • Experience working in a call center or high‑volume service environment.
  • The ability to manage competing priorities in a fast‑paced setting.
  • Experience collaborating with health care systems, community‑based organizations, and multidisciplinary partners.
  • Knowledge of crisis intervention techniques and risk assessment.
  • Experience working with community agencies and advocating for older adults and people with disabilities.
  • Excellent interpersonal and communication skills, with the ability to interact with a diverse client population.
  • A Master’s degree in Social Work or a closely related behavioral health or human services field is highly desired, but not required.
Minimum Qualifications

Experience and

Education:

  • Any combination of training and experience that would provide the opportunity to acquire the knowledge and abilities listed. Normally, a Master’s Degree in social work, or a Master’s Degree from a two‑year counseling program (e.g., marriage and family therapy, clinical counseling, mental health counseling, addiction counseling, or counseling psychology) and at least two years of experience comparable to an Adult and Aging Social Worker.
  • Or, graduation from college with a Bachelor’s Degree in social work, sociology, psychology, or another behavioral science and at least four years of experience comparable to an Adult and Aging Social Worker, or two years as a Social Worker Supervisor overseeing cases involving older or dependent adults and/or people with disabilities or complex medical conditions.
  • Or, academic coursework at a college or university in social work, sociology, psychology, or another behavioral science and at least seven years of experience comparable to an Adult and Aging Social Worker.
License:
Possession of a valid driver’s license at the appropriate level, including special endorsements as required by the State of California, may be required depending on assignment to perform the essential job functions of the position. (
