Licensed Practical Nurse (LPN) Responsibilities
  • Admissions, Discharge and Transfers of residents.
  • Medication requisitions with pharmacy and physicians.
  • Medication administration.
  • Follow infection control, emergencies, and other center policies.
  • Collaborate with resident’s interdisciplinary team to provide the best outcomes.
  • Identify and report all abnormal changes in residents’ condition to proper personnel per center policy.
  • Work under direct supervision in accordance with the state‑specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
  • Deliver nursing care to patients/residents requiring long‑term or rehabilitative care.
  • Collect patient/resident data, make observations, and report pertinent information related to the care of the patient/resident.
  • Implement the patient/resident plan of care and evaluate the patient/resident response according to state‑specific regulations.
  • Direct and supervise skilled nursing care given by other nursing personnel in selected situations.
  • Maintain knowledge of necessary documentation requirements.
  • Maintain knowledge of equipment set‑up, maintenance, and use (monitors, infusion devices, drain devices, etc.).
  • Maintain confidentiality and patient/resident rights regarding all patient/resident personnel information.
Licensed Practical Nurse (LPN) Qualifications
  • LPN (Licensed Practical Nurse) licensed by the State.
  • CPR Certification required.
  • Two years prior nursing experience preferred.
