Part-Time Healthcare Service Coordinator

Coordinate patient care schedules for Bayshore Health Care using customer service, communication, and attention to detail in an in-office setting. This part-time position requires evening and weekend flexibility.

As a Client Service Coordinator, you will connect patients, families, staff, and funder organizations. You will process referrals in Procura, assess service needs, and assign care providers. The position requires at least 15 hours weekly, with additional hours based on business needs.

• Communicate professionally with patients, families, staff, and funders
• Process referrals and updates in the Procura EMR
• Assess service needs with supervisors, employees, and patients
• Assign providers according to client needs and availability
• Maintain records and address patient concerns promptly

• Secondary School Diploma
• Two years of related customer service experience
• Strong written and oral English communication skills
• Excellent Windows, Microsoft Office, internet, and EMR skills
• Reliable transportation and evening or weekend availability
