You will be responsible for assessing clients' abilities in their homes and communities, developing tailored plans, and ensuring effective communication with the healthcare team. This full-time position emphasizes collaborative approaches, effective documentation, and holistic assessment techniques to support client independence and support systems.
Key Responsibilities:
• Conduct assessments on client independence and needs
• Collaborate with the healthcare team for optimal care
• Maintain thorough documentation of client progress
• Recommend and guide home modification tools
• Educate clients on therapy goals and expectations
Requirements:
• Master’s in Occupational Therapy or equivalent
• Current registration with the College of Occupational Therapists
• Minimum 1 year of experience in a healthcare role
• Excellent verbal and written communication skills
• Ability to adapt to varied client needs
