Hepatitis C Case Manager - Ontario

Join North Lambton Community Health Centre as a full-time Case Manager focusing on Hepatitis C care. Provide education, advocacy, and practical support to clients in a dynamic healthcare setting.

This one-year contract role involves a commitment to helping individuals navigate testing, treatment, and follow-up for Hepatitis

C. You will be part of an interdisciplinary team dedicated to client-centered case management and outreach, identifying needs and barriers to care. Collaboration with community services and support for clients and their families is essential for this position.

Key Responsibilities:

• Conduct outreach to individuals at risk of Hepatitis C

• Develop and execute personalized support plans

• Advocate for clients’ health and social service needs

• Maintain accurate records in line with NLCHC policies

• Provide education on Hepatitis C prevention and treatment

Requirements:

• Diploma or degree in relevant field required

• Minimum of three years in community-based case management

• Strong understanding of harm reduction and trauma-informed care

• Experience with electronic medical records systems

• Commitment to culturally safe and inclusive practices
