This temporary full-time role focuses on geriatric assessments, crisis prevention, and treatment implementation. Reporting to the Manager of Client Services, you will collaborate with caregivers and develop partnerships with long-term care homes. Bilingual candidates are preferred to enhance communication and care delivery.
Key Responsibilities:
• Conduct geriatric assessments and develop treatment plans
• Liaise with other support providers for continuity of care
• Implement individualized treatment plans for clients
• Plan crisis prevention strategies to avoid hospitalization
• Educate long-term care providers on best practices
Requirements:
• Bachelor of Science in Nursing degree
• Current registration with Ontario College of Nurses
• Valid Ontario Driver’s License and vehicle access
• Proficiency in both official languages preferred
• Experience in psychiatric/mental health nursing is an asset
