Key Responsibilities:

• Monitor nutrition risks for all residents consistently

• Perform RAI assessments and dietary interventions

• Develop and adjust personalized nutrition plans

• Provide nutritional education and training

• Support continuous quality improvement initiatives

Requirements:

• Must be registered with the College of Dietitians of Ontario

• Demonstrated ability to work in a multidisciplinary setting

• Strong written and verbal communication skills

• Familiarity with EMR and other software tools

• Valid proof of liability insurance and background checks
