About the Role

The Occupational Therapist provides comprehensive, person-centred care within a primary care and community health setting, supporting individuals across the lifespan to participate in meaningful daily activities and achieve optimal health and independence. Working as part of an interdisciplinary team, the Occupational Therapist addresses both physical and mental health needs through assessment, intervention, health promotion, disease prevention, rehabilitation, and community development initiatives.

This role focuses on helping clients improve function, safety, mobility, and quality of life by addressing the interaction between the person, their environment, and their daily occupations. The Occupational Therapist supports a diverse client population, including seniors, children, individuals with chronic diseases, disabilities, mental health concerns, and those requiring assistance with aging in place.

Key responsibilities include conducting assessments; developing and implementing individualized intervention plans; providing education and support to clients, families, and caregivers; completing home visits and community outreach activities; supporting falls prevention initiatives; prescribing adaptive equipment and assistive devices; facilitating access to disability benefits and accommodations; and collaborating with internal and external partners to coordinate care and improve health outcomes.

The Occupational Therapist also contributes to program development, quality improvement initiatives, health promotion activities, case conferences, data collection and evaluation, policy development, and community-based programming. The position requires strong clinical judgment, the ability to work independently in community settings, and a commitment to collaborative, interdisciplinary practice.

This position is ideal for an experienced Occupational Therapist who is passionate about community-based care, promoting independence, and improving health outcomes through holistic, occupation-focused practice.

Qualifications & Experience
  • Master's or Bachelor's Degree in Occupational Therapy.
  • Current registration and good standing with the College of Occupational Therapists of Ontario.
  • Minimum three years of Occupational Therapy experience in a community-based setting.
  • Experience working with seniors, low‑income populations, and culturally diverse communities.
  • Demonstrated ability to work effectively within an interdisciplinary healthcare team.
  • Strong assessment, clinical documentation, care planning, and case management skills.
  • Proficiency with electronic medical records and Microsoft Office applications; experience with Telus PS Suite EMR is an asset.
  • English is essential; bilingualism (French/English) and additional languages are considered assets.
Responsibilities
  • Conduct comprehensive, client-centred occupational therapy assessments (physical, cognitive, mental health, and environmental factors)
  • Develop and implement individualized, occupation-based intervention plans
  • Provide home and community-based assessments, including home safety and fall risk evaluations
  • Deliver rehabilitation, health promotion, and disease prevention interventions
  • Support chronic disease self-management (e.g., fatigue, pain, and activity management strategies)
  • Prescribe and recommend adaptive equipment, assistive devices, and home modifications
  • Provide education and support to clients, families, and caregivers
  • Support system navigation, including disability benefits and workplace/school accommodations
  • Participate in interdisciplinary case conferences, care planning, and collaboration with internal/external partners
  • Maintain accurate, timely clinical documentation and electronic medical records
  • Contribute to program development, community outreach, and health promotion initiatives
  • Participate in quality improvement activities, data collection, and evaluation activities
