As the Resident & Family Coordinator, you will facilitate a smooth integration for new residents. Your responsibilities include overseeing applications and tours, personalizing resident spaces, and coordinating care conferences. Collaboration with multidisciplinary teams and acting as a liaison between families and staff will be essential for maintaining high care standards.
Key Responsibilities:
• Manage admissions processes and home tours
• Assist families in transition and moving processes
• Facilitate annual care conferences with the care team
• Provide psycho-social support and collaborate on palliative care
• Maintain communication channels with families and the community
Requirements:
• Registered Social Service Worker (RSSW) designation
• Experience in healthcare or long-term care settings
• Strong interpersonal and communication skills
• Ability to manage sensitive situations
• Familiarity with long-term care legislation and standards
Support new residents at Oakcrossing and enhance their transition experience through compassionate communication and coordination.
