In this temporary role covering maternity leave, you'll work with individuals living with persistent mental illness, providing tailored counselling and rehabilitation strategies. Your contributions include conducting assessments, developing care plans, and offering crisis interventions. Engage with clients in their communities, making a real impact on their recovery journeys.
Key Responsibilities:
• Provide advocacy, support, and counselling to clients
• Develop personalized recovery-oriented care plans
• Support clients in accessing physical health care
• Facilitate community-based services and crisis responses
• Maintain thorough clinical documentation according to standards
Requirements:
• Bachelor's degree or equivalent in a related field
• 3-5 years of community mental health experience
• Knowledge of the Ontario Mental Health Act and recovery practices
• Strong organizational and interpersonal communication skills
• Access to a reliable vehicle and valid driver's license
