In this permanent, full-time role, you will oversee day-to-day operations of wellness and family practice teams. Your responsibilities include executing health initiatives that drive patient-centered care, while ensuring the unique needs of communities are respected. The ideal candidate has a Bachelor’s degree in health and significant leadership experience in a health care setting.
Key Responsibilities:
• Manage operations across wellness and chronic disease teams
• Implement primary health care initiatives and programs
• Collaborate with interprofessional teams on performance standards
• Facilitate community-based project development and monitoring
• Ensure compliance with organizational priorities and health determinants
Requirements:
• Bachelor’s degree in health discipline or related field
• 3-5 years of progressive leadership experience
• Knowledge of primary health care and health promotion
• Strong interpersonal, communication, and project management skills
• Valid driver’s license and reliable vehicle for travel
