Key Responsibilities:
• Collaborate with clinic staff to identify patients needing support
• Conduct needs assessments for patients with social barriers
• Facilitate patient connections to community resources and programs
• Track referrals and evaluate program impacts
• Attend community events to build partnerships
Requirements:
• 2+ years in healthcare or community development
• Understanding of Indigenous health and social determinants
• Excellent communication skills across diverse populations
• Knowledge of community resources in northern Saskatchewan
• Valid driver's license with a reliable vehicle
