Elevate patient care sharply as a Clinical Pharmacist at Mackenzie Health. Contribute through drug therapy monitoring, pharmacokinetic consultations, and patient education to promote effective medication use.
As part of a multidisciplinary team, you will assess, plan, implement, and evaluate drug therapy, ensuring safe and quality patient care. Your expertise in pharmaceutical care will help identify, resolve, and prevent drug-related problems. In this role, you will also lead and supervise Pharmacy Technicians, students, and volunteers, fostering an inclusive and safe environment.
Key Responsibilities:
• Assess and evaluate drug therapies effectively
• Educate patients to promote medication autonomy
• Apply care models to prevent drug-related issues
• Conduct drug utilization reviews for cost-effective care
• Supervise Pharmacy Technicians and interns
Requirements:
• Bachelor Degree in Pharmacy
• Licensed with the Ontario College of Pharmacists
• Previous hospital experience is required
• Empathy in patient interactions
• Ability to comply with safety policies
