Join Humber River Health as a Nurse Practitioner in our Nurse Led Outreach Team. Focus on high-quality care and efficient patient transitions from hospital to Long Term Care settings.
This full-time role requires clinical expertise in supporting residents in Long Term Care homes. Collaborating with multidisciplinary teams, you will help streamline hospital discharges, reduce emergency department visits, and support optimal health outcomes. Your contributions are pivotal in fostering connections between hospitals and LTC facilities, using evidence-based practices to enhance care delivery.
Key Responsibilities:
• Provide comprehensive care in Long Term Care settings
• Collaborate with healthcare teams for discharge planning
• Train LTC staff on acute treatment protocols
• Integrate best practice guidelines into patient care
• Evaluate program outcomes and suggest improvements
Requirements:
• Master’s in Nursing/Nurse Practitioner or related degree
• Current Ontario College of Nurses registration as NP
• 1-3 years of NP experience required
• Previous LTC experience preferred
• Strong communication and interpersonal skills
