As a Transitional Registered Nurse, your primary responsibilities include conducting participant assessments during hospital stays and facilitating post-discharge care coordination. With three years of relevant experience preferred, you'll collaborate with an interdisciplinary team to create and implement personalized transition care plans aimed at preventing readmissions. This role emphasizes the importance of comprehensive support for older adults.
Key Responsibilities:
• Assess risks for post-discharge complications
• Coordinate participant care in facilities
• Implement individualized care transition plans
• Participate in interdisciplinary team meetings
• Provide ongoing education for participants and caregivers
Requirements:
• California RN license, active and unrestricted
• 3+ years of experience preferred
• CPR/BLS certification must be current
• Valid CA driver’s license required
• Alzheimer’s Certification is a plus
