Your role involves intricate collaboration with physicians and interdisciplinary teams to create tailored care plans that prioritize the dignity and needs of elderly participants. Contribute to program development and enhance operational workflows within a compassionate care environment.
Key Responsibilities:
• Conduct comprehensive psychiatric assessments and treatment plans
• Optimize clinical workflows and lead team training
• Mentor psychiatric nurse practitioners and support clinicians
• Oversee patient care in skilled nursing facilities
• Participate in quality improvement initiatives and audits
Requirements:
• Accredited Master’s in Nursing with psychiatric focus
• Certified as a Psychiatric Mental Health Nurse Practitioner
• Active nursing license with CDS Certification, NPI and DEA
• Four years experience in elder care required
• Desired two years in PACE model leadership
