In this role, you will support client health and outcomes through collaboration with the Care Designer and care team. Your responsibilities will include conducting assessments, administering medications, and providing guidance to clients and families. With a focus on accuracy and communication, you will play a crucial role in maintaining high standards of care.
Key Responsibilities:
• Deliver nursing services in home and community settings
• Conduct assessments and develop/update care plans
• Administer medications and treatments as authorized
• Identify changes in client conditions and coordinate follow-ups
• Provide health teaching and support to clients and families
Requirements:
• Current RN registration in good standing
• Current CPR certification and First Aid as needed
• Satisfactory Criminal Record Check completion
• Strong verbal and written communication skills
• Valid driver's license and access to a reliable vehicle
