Trillium Health Partners is seeking a Registered Practical Nurse for the McCall Centre, part of the Queensway Health Centre. In this role, you will be responsible for providing comprehensive nursing care, conducting assessments, and developing personalized treatment plans for patients. Utilize critical thinking skills and collaborate with our inter-professional teams to enhance patient outcomes and support rehabilitation efforts.
Key Responsibilities:
• Manage nursing care for assigned patients
• Develop goal-oriented treatment plans
• Educate patients and families on care processes
• Facilitate discharge planning and evaluations
• Participate in quality improvement initiatives
Requirements:
• Graduate of a recognized School of Nursing
• Current RPN certification from College of Nurses of Ontario
• Recent experience in inpatient medical and rehab settings
• BCLS certified
• Evidence of ongoing professional development
