In this full-time position, you will collaborate with multidisciplinary partners to assess patients' needs and avoid unnecessary hospital admissions. Your contributions are pivotal in facilitating early discharges and in providing innovative, patient-focused care. Key Responsibilities:
• Conduct complete frailty assessments for elderly patients
• Assist in creating effective management plans
• Coordinate safe discharges and avoid unnecessary admissions
• Work closely with community and acute care departments
• Provide clinical leadership within the team
Requirements:
• Current NMC UK registration required
• Significant post-registration experience in relevant fields
• Proficient in clinical assessments and decision-making
• Strong organizational and communication skills
• Advanced qualifications in clinical practice or frailty
