In this nurse role, you will work closely with physicians to deliver high-quality patient-centred care until March 31, 2028. Your tasks will include medication management, EMR documentation, and supporting chronic disease patients. This innovative collaboration aims to improve healthcare outcomes in Saskatchewan.
Key Responsibilities:
• Provide clinical care aligned with professional standards
• Manage medication reconciliation and documentation
• Coordinate with health-care providers for comprehensive care
• Assist with chronic disease education and management
• Facilitate patient referrals to community resources
Requirements:
• Active registration and standing with CRNS
• Completion of an accredited Bachelor of Science in Nursing
• Familiarity with chronic diseases, such as diabetes
• Strong EMR proficiency, preferably with Accuro
• Commitment to ongoing professional development
Provide valuable nursing services while advancing healthcare in Saskatchewan at Acadia Medical Association.
