This impactful position encompasses assessing family needs, developing personalized support plans, and maintaining a safe travel experience for children. You will assist families with accessing services in health, education, and social resources, while also offering crisis intervention when needed.
Your role includes collaboration with community organizations and conducting home visits to monitor each family's progress.
Key Responsibilities:
• Build relationships with families for needs assessment
• Provide guidance related to health and education
• Offer safe and reliable transportation for activities
• Conduct home visits to monitor family progress
• Maintain accurate records of provided services
Requirements:
• Experience with children or families is preferred
• Excellent interpersonal and communication skills
• Demonstrated empathy and cultural sensitivity
• Experience in education, counseling, or social work is a plus
• Criminal Record Check required for suitability
