Social Worker with SHN's Interprofessional Team

Be a key member of the Scarborough Health Network as a part-time Social Worker within the Interprofessional Primary Care Team. This role focuses on providing comprehensive social work services to support patient health outcomes on weekdays and weekends.

Eligible candidates should possess an MSW degree and registration with the Ontario College of Social Workers. Your responsibilities include assessing social factors affecting health, facilitating community resource access, and engaging in interprofessional collaboration to optimize patient care. You'll be pivotal in promoting patient independence and advocating for their needs.

Key Responsibilities:

• Provide essential social work services within a clinical setting

• Participate in developing patient care plans

• Use treatment modalities for family and caregiver support

• Engage with community resources for patient access

• Monitor and document care and treatment outcomes

Requirements:

• Master of Social Work (MSW) required

• Registration with the Ontario College of Social Workers

• Experience in primary-care or community settings

• Excellent interpersonal and crisis intervention skills

• Commitment to SHN's values and ethical practices
