Position: Case Manager – Community Outreach Resource Team (CORT)

Sanctum Care Group's Community Outreach Resource Team (CORT) program is seeking a Case Manager. CORT is a mobile, rapid-response team that provides intensive, wrap-around support to people living with HIV who are also experiencing addictions, mental health concerns, and other psychosocial challenges. The goal is to improve health and social outcomes through quick intervention and coordinated case management.Temporary position until March 31, 2027, with potential for renewal.Key

Duties

  • Support people living with HIV affected by mental illness, addictions, homelessness, poverty, and related barriers.
  • Coordinate psychosocial and medical care plans and connect clients with appropriate services.
  • Work closely with health providers to support clients with complex needs.
  • Assist clients in addressing barriers such as housing, income, and addictions.
  • Provide harm-reduction-based addiction support and referrals.
  • Offer therapeutic support to clients dealing with multiple challenges.
  • Assist with identification documents and basic legal needs.
  • Transport and accompany clients to medical, legal, and social appointments.
  • Maintain accurate, timely documentation as required.
Qualifications
  • Degree in Social Work, Addictions, or related field.
  • Experience in Mental Health and Addictions.
  • Cultural awareness of First Nations issues.
  • Strong knowledge of community and housing resources and advocacy skills.
  • Ability to build effective relationships with diverse clients and community partners.
  • Understanding of Trauma-Informed Care, Harm Reduction, and HIV-related issues.
  • Strong communication, conflict resolution, and interpersonal skills.
  • Valid drivers licence and access to a vehicle.
