Make a difference as a Care Coordinator at Sanctum 1.5, specializing in support for pre/postnatal women affected by HIV/AIDS. You’ll empower clients through comprehensive care coordination and therapeutic counseling.

In this dynamic role, you will case manage clients and collaborate closely with community partners to provide targeted mental health, addiction treatment, and psychosocial support. Your focus will be on maximizing positive outcomes for both clients and their infants, alongside enhancing parenting skills in both home and community settings.

Key Responsibilities:

• Manage client cases and chart progress systematically

• Connect clients to necessary community agencies

• Engage clients actively in Sanctum and local communities

• Provide tailored support and facilitate detailed case planning

• Ensure resident safety and contact authorities if needed

Requirements:

• Diploma or certificate in social work or related field

• Cultural sensitivity and understanding of familial HIV contexts

• Knowledge of neonatal abstinence syndrome

• Familiarity with child welfare issues in vulnerable populations

• Valid driver’s license and vehicle availability
