The Case Manager will support individuals navigating crises by understanding their needs and connecting them with resources. With at least three years of experience and a relevant diploma, you'll assess client situations, document comprehensive case files, and recommend assistance.

Your role will involve actively engaging clients in their recovery process while collaborating with a dedicated team.
