Job Type: Full Time

• Assess the social and emotional needs of residents and develop individualized care plans to meet those needs
• Provide counseling, guidance, and support to residents and their families to address concerns and improve overall well-being
• Coordinate with interdisciplinary teams to ensure resident-centered care and promote positive outcomes
• Facilitate group activities and therapies to enhance socialization and engagement
• Maintain accurate records and documentation of resident interactions and care plans
