Nurse Practitioner, Community‑Based Medicaid
Mass General Brigham Medical Group Northern Massachusetts, Inc. is a not‑for‑profit organization supporting patient care, research, teaching, and community service. It is launching a Medicaid‑focused wrap‑around clinic that will increase access to high‑quality care for approximately 1,600 ACO patients. To support the launch, the organization is hiring four Advanced Practice Providers to deliver care within the community at patient homes and other settings.
These APPs will see patients in the Northern Region with urgent needs in the Haverhill/Lawrence, Lynn/Salem, and Saugus/Medford/Revere/Chelsea areas.
The role is a full‑time, 40‑hour position comprising home‑based and community‑based care with some in‑clinic care. Each provider will be assigned a panel of approximately 400 patients and will collaborate with a physician team member and interdisciplinary providers.
Job SummaryThe Nurse Practitioner is a licensed provider responsible for the assessment and management of various patient populations in ambulatory, inpatient, operative, and/or procedural settings, including diagnostic and therapeutic interventions, development of appropriate plans of care, and ongoing evaluation. The NP may also perform additional duties, such as precepting a small group of learners.
Essential Functions- Provides direct care, counseling, and teaching to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
- Performs complete histories and physical examinations.
- Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
- Develops appropriate plans of care and follow‑up based on diagnostic, laboratory, and physical examination findings.
- Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
- Consistently provides high quality and timely documentation including admission and progress notes, procedure notes, operative notes, and discharge summaries.
- Performs bedside procedures as are appropriate to the patient population.
Education
Master's Degree in Nursing required.
Experience
2 years of licensed nurse practitioner experience required.
Previous APP experience with the Medicaid ACO population in a primary care/community health setting is strongly preferred.
Licenses and Credentials
Class D Passenger Vehicle Driver's License (state license) – preferred.
Registered Nurse (RN – state license) – preferred.
Basic Life Support (BLS Certification) – preferred.
Nurse Practitioner (NP) / Advanced Practice Registered Nurse (APRN) – preferred.
Remote Type – Hybrid.
