Schedule:

Part-Time with weekend rotation

Are you a licensed Physical Therapy Assistant ready to make a difference directly in patients’ homes? Successful applicants will email Nikky at with a summary of their background and what inspires them to provide one-on-one therapy in a home setting.

• 30 years of trusted service delivering patient-centered care across 21 counties in Minnesota
• Flexible scheduling with full or part-time options and weekend rotation
• Supportive leadership with ongoing mentorship from dedicated PT supervisors
• Competitive compensation and mileage reimbursement

• Implement personalized treatment plans developed by a licensed Physical Therapist, helping patients regain strength, range of motion, and independence
• Deliver hands‑on care including therapeutic exercises, mobility training, gait education, and adaptive strategies to support recovery
• Document all interventions and patient responses in EMR to track clinical progress
• Educate patients and caregivers on home exercise routines and fall prevention strategies, empowering them with confidence
• Collaborate regularly with the supervising PT and interdisciplinary care team to ensure holistic patient care

• Licensed Physical Therapy Assistant in the state of Minnesota
• Valid driver’s license, reliable vehicle, and current insurance
• Strong communication skills, with the ability to educate and motivate patients
• Home care experience is a plus—but we will train the right individual passionate about making a difference
