Registered Nurse (RN) Case Manager Responsibilities

* Oversee and manage an assigned nursing home household with a strong focus on person-centered care

* Guide and support household staff in meeting the physical, emotional, and psychosocial needs of residents

* Administer medications and treatments in accordance with physician orders and facility policies

* Coordinate resident admissions, discharges, and transfers to and from the household

* Complete the Resident Assessment Instrument (RAI), including MDS assessments, CAAs, triggers, and individualized care plans for assigned residents

* Perform nursing skills requiring RN licensure, including IV therapy and blood draws

* Process, transcribe, and implement physician orders in compliance with established policies and procedures

* Maintain current knowledge of emergency protocols and respond effectively to emergent situations
