Care Partners is expanding its dynamic Rehabilitation Team and seeks a qualified Physiotherapist to provide community-based assessments and develop effective Care Plans. Your responsibilities include collaborating with an interdisciplinary team and establishing measurable goals with patients. This role emphasizes independence and promotes a healthy patient environment.

Key Responsibilities:

• Conduct professional assessments of patients in their homes

• Develop and maintain effective Care Plans

• Collaborate with a care team on measurable goals

• Provide guidance for workplace Health and Safety

• Perform other related duties as required

Requirements:

• Master’s Degree in Physiotherapy from an accredited institution

• Registration with the College of Physiotherapists of Ontario

• Strong assessment and treatment skills

• Reliable vehicle for travel

• Excellent communication abilities in English
