Position: FJ Davey Home Nurse Practitioner Role

In the role of Nurse Practitioner, you will support residents with complex health needs and work collaboratively within a clinical team. Your focus will be on conducting health assessments, evaluating care interventions, and documenting resident progress. The ideal candidate will possess advanced clinical knowledge and a passion for improving health outcomes in long-term care settings.

Key Responsibilities:

• Conduct health assessments on admissions and annually

• Evaluate and adjust care plans in collaboration with families

• Interpret diagnostic tests and follow up on results

• Maintain accurate documentation of care strategies

• Mentor clinical staff and promote quality improvement practices

Requirements:

• Registration with the College of Nurses in Extended Class

• Experience in Gerontology preferred

• Understanding of long-term care health issues

• Proven leadership and collaborative skills
