Key Responsibilities   Conduct scheduled community visits for patient care
Provide ethical and secure nursing services
Assess individual patient needs using appropriate tools
Monitor and adjust care plans as necessary
Maintain professional patient records accurately
Requirements   CNO registration as a Registered Practical Nurse
Valid driver's license and access to a vehicle
Experience in community care preferred
Reliability and flexibility are essential
Willingness to cover on-call duties after hours
