Key Responsibilities:

• Conduct comprehensive patient assessments

• Develop and monitor personalized care plans

• Keep professional records up to date

• Provide required on-call care as needed

Requirements:

• Registered Nurse with CNO standing

• Driver's license and vehicle access required

• Community care experience preferred

• Reliability and willingness to travel
