Key Responsibilities:

• Execute scheduled community nursing visits

• Provide safe and effective nursing care

• Assess patient needs using proper tools

• Update and manage patient records accurately

• Collaborate closely with patients to foster trust

Requirements:

• Must be a Registered Practical Nurse with CNO registration

• Valid driver's license and vehicle required

• Community care experience is an asset

• Willingness to travel for patient visits

• Required on-call availability for patient safety
