Key Responsibilities:

• Assess patient needs using appropriate tools

• Monitor and implement individualized care plans

• Build collaborative relationships with patients

• Keep detailed records of patient encounters and care

• Commit to traveling to various patient locations

Requirements:

• Current CNO registration as a Registered Nurse

• Valid driver's license and vehicle access needed

• Community care experience is an asset

• Must be reliable and flexible for on-call duties

• Passion for enhancing patient quality of life
