As a Family Support Worker, you will conduct assessments, create goal-oriented service plans, and ensure effective communication with families. Your collaboration with health and social teams will help provide comprehensive assistance to community members.

Key Responsibilities:

• Deliver personalized family and member support

• Oversee case coordination and management

• Engage in community wellness programs and initiatives

• Implement Jordan's Principle in service delivery

• Maintain accurate case documentation and reporting

Requirements:

• Degree or diploma in social work or related field

• Minimum two years of experience in social services

• Strong familiarity with Indigenous community dynamics

• Valid Ontario driver’s license and reliable vehicle

• Willing to travel and adapt to flexible schedules
