Position Summary: This position is ideal for a Nurse Practitioner (AGNP or CNP) who values flexibility, relationship‑building, and clinical growth. Whether you’re wound‑care certified or not, we’ll cover the cost of certification so you can enhance your impact in this critical area of care.
Key Responsibilities- Deliver hands‑on primary care and wound management in senior living settings.
- Participate in chronic care management, palliative care planning, and clinical coordination.
- Collaborate with interdisciplinary teams, family members, and facility staff.
- Ensure timely documentation and professional communication.
- Licensed Nurse Practitioner in Minnesota (AGNP preferred).
- Background in geriatric care, internal medicine, or primary care.
- Must have a valid driver’s license and reliable vehicle.
- Interest in or experience with wound care and older adult health.
