Promote, maintain, and improve individual and community health by assisting individuals and communities to adopt healthy behaviors for the purpose of preventing the spread of HIV and other sexually transmitted diseases. Collect and analyze data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies and environments.

May require occasional daytime travel throughout the eight county district (Bryan, Camden, Chatham, Effingham, Glynn, Liberty, Long, McIntosh) with a heavy concentration on Chatham County. Will require incumbent to perform community outreach and HIV testing events, locate and counsel HIV/STD clients and contacts in the community, and discuss sensitive topics with groups of various sizes containing people from a variety of backgrounds.

All Coastal Health District, County Health Department employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Dispensing site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.

Responsibilities
  • Provides HIV/STD health education to individuals and community members in various locations throughout the eight‑county area of the Coastal Health District 9‑1 (Bryan, Camden, Chatham, Effingham, Glynn, Liberty, Long, McIntosh).
  • Determine level of education activities to suit the individual and community group needs and wants.
  • Provides HIV Testing & Counseling in the community to a wide range of at‑risk and targeted populations to ensure testing goals are met.
  • Distributes condoms and education materials to individuals and community groups as requested.
  • Work to strengthen community ties with the program and maintain relationships with a wide variety of groups.
  • Assess individual and community needs for health education plans effective health education activities, implement health education programs, communicate health and health education needs and serves as a resource person in health education. Collaborates with health specialists and civic groups to ascertain community health needs, determine availability of services, and to develop goals.
  • Conducts community surveys to ascertain health needs, develop desirable health goals, and determine availability of professional health services.
  • Plan and coordinate delivery of intervention activities.
  • Prepare and submit monthly service reports for the Coastal Health District and the State Health Office of HIV/AIDS.
