• Coordinate and direct the provision of home nursing care according to physicians' orders and agency policies and procedures.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conduct home visits and attend periodic office meetings.
• Possess reliable transportation, a current driver's license, and agency-required liability insurance.
• Manage an average of 5-8 patients per day, equating to roughly 30 points per week.
• Handle various cases, including Start of Care, Wound Care, and Follow-up Visits.
• Discuss weekend and on-call requirements as they vary by location.
• Cover a territory with a mileage radius of up to 75 miles from the assigned home clinic, with mileage reimbursed at the IRS standard rate.
• Understand that each type of visit and activity (documentation, travel, etc.) holds a different point value.
• Complete all documentation within 24 hours of the appointment with the patient.
• Be open to traveling up to 75 miles away from the assigned care center and participate in on-call rotations every 3-4 weeks.
• Attend weekly meetings at the clinic and adhere to productivity expectations of 32 points per week.
• Be proficient in OASIS and have used the Home Care Home Base EMR within the last 6 months or complete a retraining course if not.
