Nurse Case Manager - Wound Care Registered Nurse
Location: Bluffton, SC
Salary: $70,000.00 – $80,000.00
Job Summary: Registered Nurse to service Wound Care patients across South Carolina. The Nurse Case Manager will visit patients, facilitate appropriate providers, evaluate patients at homes or long‑term care facilities, and initiate telemedicine visits. Services are delivered in compliance with all applicable laws, regulations, and policies within the scope of practice.

The team collaborates closely with facilities and families to ensure accessible, coordinated, and compassionate care.
Responsibilities- Facilitate receiving all medical records from the patient’s primary provider and specialists.
- Review medical records.
- Complete consents with patients.
- Enroll patients in Care Management when eligibility criteria are met.
- Initiate a Care Management Plan of Care for eligible patients.
- Capture all diagnoses at the highest specificity, create gaps, and ensure acceptance.
- Complete Annual Wellness Visits (AWVs) for provider review.
- Complete cognitive impairment screenings.
- Complete Social Determinants of Health (SDoH) assessments and/or screenings.
- Complete Advance Care Plans (ACPs) for patient-provider review.
- Evaluate for home health, hospice, palliative care, or consults with Your Health Specialty Division.
- Evaluate for RPM devices, resources, or tools to improve quality of life.
- Communicate and coordinate care across the care team.
- Reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments.
- Provide post‑discharge education.
- Evaluate for adaptive equipment and durable medical equipment (DME).
- Evaluate for safe environment.
- Evaluate acute conditions or follow‑up from previous visits.
- Document and log Care Management activities accurately; keep care plans up to date with the care team.
- Travel to patients’ homes, long‑term care facilities, or other service sites daily; reliable transportation is required.
- Licensed nurse; RN preferred; license in good standing with the appropriate board or issuer.
- Minimum of three (3) years of clinical experience preferred.
- Experience in community settings preferred.
- Strong communication and collaboration skills with interdisciplinary teams, patients, and caregivers.
- Strong written and verbal skills.
- Basic computer knowledge.
- Demonstrated leadership and managerial ability.
- Interpersonal and communication skills under all conditions.
- Ability to foster a cooperative work environment.
- Team player who can manage multiple responsibilities and exercise sound judgment.
- Flexible schedule and willingness to travel between offices, facilities, and other sites; must be a licensed driver with a vehicle that is insured and in good working order.
