• Provide direct nursing care under the supervision of the RN and healthcare team
• Assist with developing, implementing, and updating resident care plans
• Administer medications and treatments within the scope of practice
• Monitor resident conditions and communicate changes to the care team
• Maintain accurate documentation and medical records
• Support resident comfort, safety, and quality outcomes

• Current Licensed Practical Nurse (LPN) license in the State of Oregon
• Recent clinical, long-term care, or geriatric nursing experience preferred
• Knowledge of infection control practices and nursing standards
• Strong communication and interpersonal skills
• Basic computer skills and ability to learn company software
• Ability to read, write, speak, and understand English
• Ability to pass required employment screenings and background checks
