Salary: $52.83 – $74.83/hour (may include evenings, weekends, and on‑call)

This role focuses on delivering comprehensive, family‑centered primary care to unattached patients, with an emphasis on health promotion, disease prevention, and equitable access to care.

• Provide comprehensive primary care across the lifespan within NP scope
• Assess, diagnose, and manage acute and chronic conditions
• Deliver preventive care, including immunizations
• Develop and implement patient‑centered care plans
• Collaborate within an interdisciplinary team
• Support marginalized populations with complex health needs
• Participate in health promotion and outreach initiatives
• Maintain clinical documentation and ensure confidentiality
• Contribute to quality improvement initiatives
• Participate in on‑call rotation and occasional home visits

• Master’s degree in Nursing
• Primary Health Care Nurse Practitioner (PHCNP) certification
• Registered in good standing with the College of Nurses of Ontario (Extended Class)
• Experience working with diverse and underserved populations
• Experience in community health or primary care settings (preferred)
