Key Responsibilities:

• Assess and monitor client needs in home settings

• Evaluate care plans based on ongoing assessments

• Ensure quality care and practice autonomy

• Inform supervisors of relevant client updates

• Participate in on-call rotations for urgent care needs
Requirements:

• RN in good standing with the College of Nurses of Ontario

• Valid First Aid or CPR Certification

• Experience in acute care or community nursing preferred

• Strong communication and documentation skills

• Valid Ontario driver’s license and reliable vehicle
