In this position, you will provide essential health services and support to marginalized populations through an intersectional lens. Collaborating with an interdisciplinary team, you will ensure that clients receive equitable access to health services, emphasizing prevention and holistic care practices. Bilingualism, especially in French, is a considerable advantage.
Key Responsibilities:
• Provide comprehensive assessments and health care services
• Initiate and oversee care for clients with chronic illnesses
• Develop health care plans in partnership with clients
• Conduct outreach and home visits for community members
• Facilitate health education and advocacy initiatives
Requirements:
• Bachelor's degree in Nursing and NP certification required
• Registered with the College of Nurses of Ontario
• Extensive community-based nursing experience preferred
• Strong communication and collaborative skills needed
• Preparedness for occasional weekend work
